The following must be included to be considered as a completed application:

• Applications must be filled out in its entirety and submitted by the due date listed on our website.
• You must submit three (3) character references. At least one of the character reference letters being from a Coach or Trainer if you are already involved in MMA.
• The character reference letters must include the reference's contact information should we have further questions.
• A video must be submitted along with references. Video must be 1-2 minutes, uploaded to YouTube or emailed to info@sammanmmafoundation.org.
• Please tell us who you are and why MMA is important to you. Highlight reels will not be accepted.
• Please be sure to set the appropriate permissions on YouTube videos, ensuring it is visible to the Foundation board members. We recommend sharing the video as 'UNLISTED'.
